For applications in Dryden, Red Lake, Sioux Lookout, Cochrane, Iron Bridge, Blind River, Nipigon and Sudbury (Hanmer):

If you are interested in applying for rental housing in Dryden, Red Lake, Sioux Lookout, Cochrane, Iron Bridge, Blind River, Nipigon and Sudbury (Hanmer) please access our Central Applicant Registry to apply online. To access a paper copy, please download the application form. Complete forms should be faxed to the number identified on the application. Alternatively you may also dial (866) 391-1061 ext. 216 or 308 and ask for the "Central Applicant Registry" line to register your application over the telephone.

If you are interested in applying to Ontario Aboriginal Housing Services for subsidized housing please download the preliminary application form and fax or mail to the applicable address or fax number.

The Preliminary Survey of Need application form consists of:


1. Preliminary Survey of Need
2. Client Affordability Analysis
3. Assets and Liabilities
4. Verification of Income

ALL PAGES MUST BE COMPLETED and returned to our office. This information will be reviewed, and we will contact you for an in-home interview once a rental unit becomes available in your area. You must provide current Income Verification before your application can proceed.
Please bear in mind that we have a large number of families on file and that the needs of the largest families living in the worst conditions must first be considered.
