The Preliminary Survey of Need application form consists of:

1. Preliminary Survey of Need
2. Client Affordability Analysis
3. Assets and Liabilities
4. Verification of Income

All of the above 4 forms are to be completed and returned to our office. This information will be reviewed, and we will contact you for an in-home interview once a rental unit becomes available in your area. You must provide current Income Verification before your application can proceed.

Please bear in mind that we have a large number of families on file and that the needs of the largest families living in the worst conditions must first be considered.
